The above image said that applicaiton can be found from http://www.hkdrc.org (香港長跑會 Hong Kong Distance Running Club).

But, the form is still unavailable to be downloaded at the site mentioned above.

BTW, I will try to convert it to doc or pdf tonight

There are many ups and downs in the race course.  I found it quite difficult to me, but the race was full of fun with amazing scene.
