146,147.... Hmmmm,I think because you ran a little too fast for the 400m and became out of breath eventually. Just to remember that interval training is to optimize VO2max. You don't need to do a sub 2 min. to achieve it. You won't reach VO2max until you pass the 2 minutes mark anyway. That means your body takes about 2 minutes to reach the point where it is operating at maximum oxygen consumption. 215 is absolutely fine as long as you do not take your recovery time longer than you need to. And try to maintain a proper pace during each interval. My own rule of thumb is I never let my HR go below 120 before the next interval. Hope this helps.
